Activity Waiver Requirement
Please complete and sign an Activity Waiver if you’re a parent or legal guardian of a child under 18 years of age who wants to participate in any of the following programs:
- PJCC Sports and Recreation programs
- Youth and Teen programs
- Trips and Tours programs
